Every Project

Every Connection Support Project is a partnership between Commissioners, Connection Support and Service Users.   Connection Support prides itself on this partnership approach with very positive feedback from Commissioners on our ability to

Set up new projects in line with the vision of Commissioners and Service Users.
Remodel projects with Service User input including positive management of reduced resources.

Formal Partnerships

Oxford Mental Health Partnership

Connection Support took a full role in the “Capable Provider Process” to create an Outcome Based Contract with Oxford Health NHS Trust, Response, Restore, Oxfordshire MIND and Elmore.

Connection Support is contributing fully to the Single Point of Access approach, which is one facet of a fully integrated set of mental health services across the statutory and third sector.   This has led to

Oxfordshire Homeless Pathway

Connection Support created partnerships with Response, Cottsway Housing and Cherwell District Council to deliver 47 units of accommodation for homeless people in ordinary homes with provision across the Districts to meet local needs.

Connect ILS – support to tenants of independent living schemes in Bucks

Connection Support is working in close partnership with a wide range of housing providers across Buckinghamshire to deliver this project.  Regular Stakeholder meetings including Commissioners, Landlords and Emergency Alarm supplier, has led to a well planned service in a changing world.  As the Commissioner noted “It is everything we hoped for and although it has been a complex transition it has been delivered really smoothly.”

Money Advice in Milton Keynes

A formal partnership between MK CAB and Connection Support to deliver advice AND  support to residents of Milton Keynes with great outcomes.

MEAM

Making Every Adult Matter has been commissioned  by Oxford City Council to ensure creative solutions are found for service users with multiple and complex needs across a range of services.  Connection Support provides the coordination function.
