Out of Hospital Oxfordshire

Our Specialist mental health workers work in a multi-disciplinary team of psychiatrists, social workers and housing officers to support people with complex mental health who are homeless, or at risk of homelessness, whilst they are in hospital and when they are discharged from hospital.

Housing First

Housing First Oxfordshire encompasses a range of different projects across the county where we provide intensive support to rough sleepers during their journey from rough sleeping to a more settled lifestyle within their own home and to improve their health and wellbeing.

Family Solutions Plus Service

We provide specialist mental health workers to parents and carers to support good mental health. We work alongside Oxfordshire County Council as part of a wider team including Specialists in Domestic Abuse, Addiction and Children’s Services to provide the right support for parents experiencing poor mental health.

The Rise and Shine Service operates in partnership with Elmore Community Services. It provides support to people working towards a specific goal that may be suitable for a brief intervention.


We help people who are sleeping rough to find more settled accommodation, to access health and mental health services and drug and alcohol support services and receive the benefits they are entitled to.

Violence Reduction Service

Connection Support Navigators provide support and advice to patients as part of the Thames Valley Police Violence Reduction Unit. The service is delivered through a team of trained volunteers we call Navigators.

Refugee Mum and Daughter

This service is specifically for Syrian Refugees resettling in the UK as part of  Central Government’s SVPRS scheme (Syrian Vulnerable Persons Resettlement Scheme).

Our Homeless Support service provides accommodation and support to single homeless people over the age of 18.

Young People’s Supported Accommodation (YPSA) provides supported accommodation in Buckinghamshire and South Oxfordshire to give a specific route for young people (16-25 years) facing homelessness.

Step Down Housing

Short-term supported housing for patients who are medically ready to be discharged from the Oxford University Hospital or Oxford Foundation Trust Hospital sites but lack suitable accommodation or home to continue their recovery.
