Acute Admissions Avoidance Service

About the Service

Through short-term funding from the winter pressure funds, the team provide support to people with a mental health diagnosis. Without support, individuals may experience a crisis leading to them becoming unwell a likely to be admitted onto acute mental health wards.

What can we help with?

To focus on practical and emotional support including:

  • Tenancy sustainment
  • Housing issues
  • Daily living skills
  • Budgeting skills
  • Benefit issues
  • Attending appointments
  • Social skills
  • Linking in with other services

To provide one-to-one support visiting clients at home, to improve confidence and promote independent living skills.

We do not provide:

  • Housing
  • Domiciliary care

For people known to community mental health and the hub.

What can I expect from the Service?

One to one support with experienced professional support staff using a person-centred strength-based approach

How do we work?

Referrals to this service are via the community mental health teams and The Hub. Once a referral is received, we aim to contact the client within 7 days to carry out an initial assessment to gain further information to assess the needs and outcomes desired by the client. We work in partnership with the client to complete a strength-based action plan and work towards their goals to achieve their outcomes. With support, we aim to avoid client crises and the need for admission to hospital.
