We are currently recruiting for a Community Practice worker to join our Prevention Matters team in South Buckinghamshire.

Prevention Matters supports people to regain confidence, and independence where people are experiencing difficulties such as getting out, remaining independent at home, feeling isolated, lacking in confidence or recovering from illness.

The impact you will have

You will empower and encourage people to access appropriate services, reconnect with family, friends or community networks, support to attend activities, increase use of technology or access any other service/resource that supports the client to live independently, with the aim of the prevention or delay for long term care intervention.

The details

Contract/Hours: Permanent, 37.5 hours per week, across Monday – Friday. Part time hours/job share will be considered.

Hours: Between 08.30am-17.00pm, Monday to Friday. On occasion we might need slight flexibility to support people to appointments.

Salary: £24,397 – £26,649 per annum, pro rata if part time, dependent on experience.

Closing date: Friday 21st February @12.00pm

Interviews: We will be holding introduction calls on Wednesday 26th February, and interviews will be on March 4th and 5th.

What we need from you

Your experience might include working, volunteering or caring for people with complex needs, mental health, substance misuse, social work, disabilities, long term health conditions, learning disabilities, older people or similar.

Wherever your experience comes from, the main aim of the role is to be able to connect with people to encourage them to access services, and to not give up. Does this sound like you?

Part of the role will also include recording information accurately, so a basic level of IT skills will be essential, i.e. responding to and writing emails, typing notes etc. Full training will be given on our internal systems.

This role supports people across South Buckinghamshire however you may travel to other areas on occasion, therefore a driver’s licence and access to a vehicle is essential.
