The Trailblazer Service works closely with local authority housing teams, health (including the Mental Health Partnership), social care and criminal justice to reach those who are at risk of homelessness and prevent it at the earliest possible point. The programme covers the whole of Oxfordshire and is in line with the Homelessness Reduction Act.
What can we help with?
Connection Support’s Trailblazer team will provide added value to statutory services that come into contact with households at risk of homelessness and work with them before they reach crisis point.
The Embedded Housing Officers will support statutory professionals to provide timely and accurate housing and homelessness advice.
We do not:
Provide accommodation directly.

How do we work?
Our Trailblazer Embedded Housing Officers work across health, criminal justice and social services to help deliver early interventions for people at risk of homelessness.
Our team work with Resettlement Workers and Probation Officers in the prison service; Sisters and Nurses in hospital wards and Health and Social Workers.
We connect up the need of the client/patient with their housing situation to improve the likelihood of a successful homelessness prevention.
