Acute Hospital Discharge

About the Service

This service provides support for people who are currently on an Acute Mental Health Ward in the Campbell Centre, Milton Keynes and are to be discharged back into the community. The service offers emotional and practical support for people to gain the skills and confidence to live independently and avoid readmission.

Practical and emotional support including:

  • Tenancy sustainment
  • Housing issues
  • Daily living skills
  • Budgeting skills
  • Benefit issues
  • Attending appointments
  • Social skills
  • Linking in with other services

To provide one-to-one support visiting clients at home to improve confidence and promote independent living skills.

We do not provide:

  • Provide housing
  • Domiciliary care

Who is this service for?

For people discharged from acute mental health wards at the Campbell Centre or step-down beds.

What can I expect from the Service?

One-to-one support with experienced professional support staff using a person-centred strength-based approach. To avoid readmission back into the hospital by building emotional resilience and confidence to empower people to make positive changes and healthier mental health.

How do we work?

We work in partnership with the client to complete a strength-based action plan and work towards achieving their goals and aspirations. With support, we aim to avoid client crises and the need for admission to the hospital.
