Out of Hospital Community Team

About the Service

The Out of Hospital Care Team operates in partnership with Oxford City Council and Cherwell District Council Housing Services. It includes The Out of Hospital Community Team which comprises five community based mental health workers (one site based in Banbury) and 2 workers embedded in mental health hospitals. The team supports people with mental and physical health challenges, to enable them to avoid frequent hospital admissions and/or homelessness.

Our team work closely with clients to help them:

  • Resettle into new accommodation
  • Address rent arrears and debt
  • Develop social engagement and access to meaningful activities
  • Address anti-social behaviour
  • Make applications for housing
  • Manage new and existing benefit claims

Who is this service for?

The service is for people who experience repeated hospital admissions due to poor mental health, and whose wellbeing is impacted by housing issues.

How do we work?

The service provides wraparound support to enable people to access new tenancies and to maintain existing ones. After initial assessments of needs, suitable clients are allocated a caseworker. Personalised, practical and motivational support is provided through regular face to face contact. Clients may need support with rent arrears and benefits, maintaining wellbeing and accessing meaningful activities and employment.

The wider Out Of Hospital Care Team is a multi-disciplinary team which includes a psychologist, occupational therapist and social workers.
