The Service

The service is for patients who are discharged from hospital who require more time to recuperate or lack suitable accommodation to return to.

Short-term supported housing (for up to eight weeks) for patients who are medically fit to be discharged from the Oxfordshire Hospitals to include Oxford University Hospital and Oxford Health Foundation Trust Hospital sites

What can we help you with?

The guest will spend a maximum of eight weeks at one of the Step Down houses with support from Case Workers onsite Out Of hospital Care Team.

During this time, Case Workers will ensure guest housing options are explored which could include a referral to or – the Homeless Pathway local housing authority, and outreach services whilst they complete their medical rehabilitation. Other support could include benefit claims, healthy living, housing and liaising with appropriate agencies

Who is this service for?

Patients who are currently in Oxfordshire hospitals.  You must be homeless or unable to return to current accommodation once you have been discharged.

How do we work?

Guests are supported to look at all housing options and support offered by Case Workers by empowering the guests to be as independent as possible in preparation for move on.  Guests will be supported to understand their local connection, to inform he local housing authority of their housing situation and look at private rented accommodation, Adult Homeless Pathway, returning home and any costs involved and how to access this if appropriate via the local housing authority

After recovery?

Once recovery has been achieved the client will be supported around housing options as appropriate but discharge to the streets may be necessary.
